| THE PROCESS The Short Version* 1. Complete a Patient Intake
The Detailed Long Version We look forward to working with you as you go through the process of becoming a Medical marijuana Patient. Your appointment is for the review of your medical condition under the Revised Code of Washington 69.51.a, and possible authorization to use and possess medical cannabis (marijuana) under strict conditions imposed by the State of Washington, for personal medical use only. The process for authorization requires that we have on file all medically relevant documentation to support your assertion that you fall within the law noted above. This means that your medical records are of utmost importance, if you are to be approved for medical cannabis use. Make sure you have any supplemental records you would like the doctor to review. Upon arrival at clinic, please check in with the front office staff. Our staff will present you with a minimal amount of intake paperwork, and will collect your cash payment of $200. Sorry we do not accept credit or debit cards. Please be ready with your Washington State Driver License or Washington State ID card, as this establishes your residency requirements. US passport or military identification cards are acceptable, however more documentation may be requested. Next you will meet with one of our staff who will review your file, check your vital signs and answer any question you may have about the law or process. Finally you will meet the doctor to review your medical records, have an examination, and an extensive consultation to review the risks and benefits of medical cannabis. At this point, if certification criteria is met, you will leave the clinic with legal patient status, including official documentation. |
